Additional Insured Form

In order to host a sanctioned IFA Event and receive an "Additional Insured" certificate for your facility/complex/site, you must complete the following form below and "SUBMIT" it to us.  DO NOT wait until the last minute to apply, as we are subject to responses from the insurance agent for this certification.
